  2. Indicate how you will be funding your account (i.e. Bank Draft or Wire Transfer)
  3. Fax the signed form to DGM Bank & Trust Inc. at (246) 431 3439 (to speed up the application process). A DGM private banker will contact you upon receipt of your application.
  4. Send the original signed copy to DGM Bank & Trust after keeping a copy for your files.

Set up a Diversified ETF Select Account

  1. Download, print, and complete the Diversified ETF Select Account Form. Please be sure to read the terms and conditions, and attach any required documents as specified by the application instructions.
  2. Indicate how you will be funding your account (i.e. Bank Draft or Wire Transfer).
  3. Fax the signed form to DGM Bank & Trust Inc. at (246) 431 3439 (to speed up the application process). A DGM private banker will contact you upon receipt of your application.
  4. Send the original signed copy to DGM Bank & Trust after keeping a copy for your files.

Set up a Strategic Mutual Fund Account

  1. Download and read the Explanatory Memorandum for all our Mutual Funds.<
  2. Download, print, and complete the Mutual Fund Account Application Form. This application will allow you to open a new Mutual Fund Account at DGM. Please be sure to read the terms and conditions and attach any required documents as specified by the application instructions.
  3. Fax the signed form to DGM Bank & Trust Inc. at (246) 431 3439 (to speed up the application process). A DGM private banker will contact you upon receipt of your application.
  4. Send the original signed copy to DGM Bank & Trust after keeping a copy for your files.

Set up a DGM Global Advisory Account

  1. Download, print and complete the Investor Questionnaire.
  2. Fax the completed questionnaire to DGM Bank & Trust Inc. at (246) 431 3439, with the original to follow by post.
  3. A DGM Bank & Trust representative will contact you upon receipt of your questionnaire.

Set up a Discretionary Investment Management Account

  1. Download, print, and complete the Discretionary Portfolio Management Application Form. Please be sure to read the terms and conditions, and attach any required documents as specified by the application instructions.
  2. Indicate how you will be funding your account (i.e. Bank Draft or Wire Transfer)
